At Mission Australia, we believe everyone deserves the opportunity to thrive. As a Case Manager, you'll work alongside people experiencing complex challenges, providing person-centred support that empowers them to achieve their goals, build independence and create lasting positive change.

Using a strengths-based and trauma-informed approach, you'll develop meaningful relationships with clients, coordinate tailored support plans and collaborate with community partners to deliver high-quality outcomes.

We are currently recruiting 3 full time role and one part time permanent roles.

Your key responsibilities will be to:

Deliver comprehensive, person-centred case management for individuals experiencing homelessness or at a risk of homelessness with complex needs
Undertake client assessments, develop holistic case plans and review progress towards agreed goals
Coordinate referrals and connect clients with appropriate internal and external support services
Provide evidence-based interventions that promote independence, wellbeing and positive outcomes
Support clients through transitions, helping them develop the skills and confidence to self-manage and access ongoing supports
Build strong partnerships with community organisations, government agencies and service providers to enhance client outcomes
Facilitate group programs, education sessions or outreach activities where required
Maintain accurate case notes, client records and reporting in line with organisational and funding requirements
Contribute to continuous improvement initiatives while supporting a collaborative and inclusive team environment
Provide mentoring and guidance to colleagues where appropriate

Requirements for Success

Relevant tertiary qualifications in Social Work, Community Services, Human Services or a related discipline or equivalent experience
Experience providing case management to people with complex needs within the community services, housing, homelessness, mental health or related sectors
Strong assessment, planning and case coordination skills
A sound understanding of strengths-based, trauma-informed and person-centred practice
Excellent communication, relationship-building and stakeholder engagement skills
The ability to manage competing priorities while maintaining high-quality documentation and reporting
A commitment to empowering people to achieve sustainable outcomes

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
