About the Opportunity
Our client is looking for a Case Manager to service the Macedonian community in Sydney.
This is a fantastic opportunity for an experienced Case Manager who is passionate about supporting people and making a meaningful difference within the community. The successful candidate will work closely with clients and their families, providing personalised support and helping them navigate appropriate services and resources.
Macedonian language skills are essential for this role, as you will be supporting members of the Macedonian-speaking community.
About the Role
As a Case Manager, you will be responsible for:
- Managing a caseload and providing ongoing support to clients
- Building strong and trusting relationships with clients and their families
- Assessing client needs and coordinating appropriate services and supports
- Developing and monitoring individualised care and support plans
- Liaising with service providers, healthcare professionals, families and community organisations
- Advocating for clients and helping them access appropriate services
- Maintaining accurate and confidential client records
- Supporting clients to achieve their individual goals and improve their quality of life
About You
We are looking for someone who:
- Has experience working as a Case Manager or in a similar community services role
- Is fluent in Macedonian and English
- Has strong communication and interpersonal skills
- Is compassionate, organised and client-focused
- Can work independently while also collaborating effectively with a wider team
- Has a strong understanding of community-based support services
- Is committed to providing culturally sensitive and person-centred support
