Bliss Nursing & Community Care is seeking an experienced and dedicated SAH Case Manager who is fluent in Turkish and confident in SAH and HCP budget planning. This role is ideal for someone who understands aged care funding, can manage complex client needs, and ensures services are delivered within each client’s allocated budget.
About the Role
As a SAH Case Manager, you will support clients to live safely and independently at home. You will be responsible for assessing needs, planning services, and ensuring all supports align with SAH/HCP funding guidelines. Your ability to communicate clearly with Turkish‑speaking clients and families is essential.
Key Responsibilities
- Comprehensive Client Assessment — Identify client needs, risks, and goals, ensuring care plans reflect their cultural, personal, and health requirements.
- Budget Planning & Management — Develop and monitor SAH/HCP budgets, ensuring all services fit within the client’s allocated funding.
- Service Coordination — Organise domestic assistance, personal care, respite, transport, and other supports based on client needs.
- Compliance & Documentation — Maintain accurate case notes, care plans, budget reports, and ensure all work meets aged care standards.
- Client & Family Communication — Provide clear guidance to Turkish‑speaking clients and families, helping them understand their funding and available services.
- Ongoing Monitoring — Review client progress, adjust care plans, and respond to changes in health or circumstances.
Required Experience & Skills
- Proven experience in SAH and HCP case management
- Strong understanding of aged care funding models and budget allocation
- Fluency in Turkish.
- Excellent communication, problem‑solving, and organisational skills
- Ability to work independently and manage multiple clients
- Compassionate, professional, and committed to high‑quality care
Pay: $35.00 – $40.00 per hour
