About the role

Join Western Health’s Pathway to Good Health program and help improve health outcomes for children and young people in Out of Home Care (OOHC).

This rewarding role combines paediatric speech pathology practice with multidisciplinary care coordination, health assessment and advocacy. You will work closely with children, carers, families and partner organisations to identify needs, reduce barriers to care and support positive developmental outcomes.

Working within a trauma-informed, culturally safe and evidence-based framework, you will contribute to health assessments, management planning and service navigation, ensuring children receive the care and support they need to thrive.

Key responsibilities

Conduct paediatric speech pathology assessments and contribute to multidisciplinary health assessments and health management plans
Provide clinical advice and secondary consultation to carers, schools, healthcare professionals and community services
Support children and young people in OOHC to access appropriate health, specialist and community services
Collaborate with internal and external stakeholders to deliver coordinated, child-centred care
Participate in outreach activities, case reviews, quality improvement initiatives and multidisciplinary meetings

About you

You are a compassionate and skilled Speech Pathologist who is committed to improving outcomes for vulnerable children and young people.

You will bring:

A tertiary qualification in Speech Pathology and eligibility for membership with Speech Pathology Australia
Experience working as a Speech Pathologist in a paediatric setting
Demonstrated experience undertaking assessments and developing individual intervention plans
Strong multidisciplinary teamwork, communication and relationship-building skills
The ability to work with professionalism, sensitivity and a strong commitment to confidentiality and child-centred care
