Full Job Description

Holistic Care SA is expanding and require a dedicated specialised support coordinator, with social work registration, and experience.

Qualifications and Knowledge

· Tertiary qualifications in a relevant field, such as Bachelor of Social Work.

· Must be registered with appropriate peak body, such as AASW Membership.

· Current valid and unrestricted driver’s licence

· Current DHS Worker Screening Clearance

· NDIS Worker Orientation Module Certificate

· Current Working with Children Check

· A good understand of NDIS and the NDIS Code of Conduct

· Understanding of NDIS Restrictive Practices Guidelines

· Work Health and Safety Act 2012 (SA) (the WHS Act)

· Work Health and Safety Regulations 2012 (SA) (the WHS Regulations) and supported Codes of Practice

· A good understanding of Equal Opportunity Principals and Practices Practice

· An understanding of cultural sensitivity

Position Summary

The SpecialisedSupport Coordinator involves connecting participants with a NDIS plan to supports and services in the community, including exploring housing options. The main purpose is to support and assist within Australia’s NDIS participant’s abilities to connect to and coordinate informal, community and funded supports in the service delivery environment. The Specialised Support Coordinator, along with the social work degree or psychology degree, has the ability to provide therapy and capacity building supports focusing specifically on social, emotional and practical barriers to wellbeing under functions such as counselling, emotional regulation and psychosocial functional capacity assessments for participants that are not associated with the specialised support coordination function.

Main Roles and Responsibilities

· Provide specialised supports to participants under the NDIS that have multiple complexities and barriers that significantly affect their daily lives.

· Provide social work therapy to participants who have required funding in their NDIS Plans to deliver psychosocial functional capacity assessments for NDIS planning and reviews.

· Provide ongoing support with complexities in liaising and working across multiple agencies such as the justice system, health, housing barriers and disability services.

· Provide employment related assessments and counselling services that align with NDIS guidelines.

· The ability to provide complex and problem-solving strategies with stakeholders and participants to assist participants meet their goals and remove barriers.

· The formatting and implementation of crisis management plans, to prevent escalation in high-risk situations, that promotes safeguarding and continuity of care for the participant.

· The provision and ability to liaise and negotiate with all service providers to ensure participants are able to achieve their NDIS goals.

· Engagement with government and non-government agencies to ensure effective support and effective provision is met to meet the participants NDIS Goals.

· Ensure Work Health and Safety guidelines are adhered to, and all industry relevant legislation and codes of conduct are complied with

· Provide NDIA reports as per NDIA requirements (such as implementation report and Quarterly reports)

· Participate in professional training and personal development

· Maintain an up-to-date knowledge of the NDIS systems and practices

· Performs other duties as assigned, including any non-billable activities within the organisation.

· Undertake work in line with the organisations policies, procedures, and practices

· Maintain accurate and up-to-date case notes on participant progress.

Skills and Experiences

· At least 2 years’ experiences within the disability sector

· Demonstrate high level interpersonal, communication and teamwork skills

· Well-developed oral and written communication skills

· Demonstrate problem solving and negotiation skills

· Ability to manage time effectively and work under pressure

· Demonstrate understanding of the changing and diverse needs of NDIS participants

· Ability to work with a high level of independence and autonomy

· Proven experience in the use of Microsoft Office including Outlook, Word and Excel

· Ability to process and prioritise a high volume of activities in a fast-paced environment

· Demonstrated ability to use a planned, structured approach to allocation of resources

Pay: $61,996.22 – $105,745.65 per year
