We are seeking an experienced and motivated NDIS Support Coordinator & Business Development Manager to join our team. This is a combined role suited to someone who has strong knowledge of the NDIS, experience supporting participants to achieve their goals, and the ability to build professional relationships and generate new business opportunities.
The successful candidate will manage a caseload of NDIS participants while also developing referral networks, building strategic partnerships, and supporting the continued growth of Care 24.
Key ResponsibilitiesNDIS Support Coordination
- Manage a caseload of NDIS participants and provide high-quality Support Coordination services.
- Assist participants to understand and implement their NDIS plans.
- Support participants to exercise choice and control over their supports and services.
- Identify and connect participants with suitable mainstream, community, funded and informal supports.
- Build participants' capacity to independently manage their supports wherever possible.
- Coordinate services between participants, families, nominees, providers, allied health professionals and other stakeholders.
- Assist participants to identify appropriate service providers based on their individual goals and support needs.
- Monitor participant plans, budgets, service utilisation and progress towards NDIS goals.
- Identify potential funding gaps, risks or changing support needs and assist with appropriate escalation or plan review processes.
- Prepare progress reports, implementation reports and other documentation required by the NDIA.
- Maintain accurate case notes, participant records and supporting documentation.
- Respond appropriately to safeguarding concerns, incidents and changes in participant circumstances.
- Ensure Support Coordination is delivered in accordance with NDIS requirements, professional boundaries, conflict-of-interest requirements and Care 24 policies.
Business Development & Referral Growth
- Develop and implement strategies to grow Care 24's participant base and service network.
- Generate participant referrals through professional networking and relationship building.
- Build and maintain strong relationships with Support Coordinators, Specialist Support Coordinators, Plan Managers, hospitals, allied health professionals, Local Area Coordinators, Case Managers, community organisations and other referral partners.
- Identify new business opportunities and assist Care 24 to expand into new service areas and regions.
- Represent Care 24 at networking events, expos, conferences and community meetings.
- Develop referral partnerships with healthcare, disability and community organisations.
- Follow up enquiries and referral opportunities in a professional and timely manner.
- Assist with presentations, capability statements, proposals and service information for prospective participants and referral partners.
- Work towards agreed monthly referral, participant acquisition and business development targets.
- Maintain accurate records of referral sources, enquiries, networking activity and business development outcomes.
Participant & Stakeholder Relationship Management
- Develop professional and trusted relationships with participants, families and stakeholders.
- Maintain regular communication with participants and relevant members of their support network.
- Work collaboratively with Support Coordinators, Plan Managers, allied health professionals, Behaviour Support Practitioners, healthcare professionals and other providers.
- Promote positive participant outcomes while maintaining appropriate professional boundaries.
- Ensure participant concerns, complaints and service issues are addressed promptly and professionally.
Operations & Service Development
- Work closely with Care 24 management to identify opportunities to improve service delivery and participant outcomes.
- Assist with participant onboarding and service commencement.
- Support the development and implementation of internal procedures relating to participant engagement and Support Coordination.
- Assist management with workforce planning where participant support requirements change.
- Contribute to organisational growth, quality improvement and compliance initiatives.
- Support a workplace culture focused on accountability, collaboration, quality and participant outcomes.
Essential Skills & Experience
- Minimum 2–3 years' experience in NDIS Support Coordination, Specialist Support Coordination, disability services, case management or a related role.
- Strong understanding of the NDIS, NDIS participant plans, funding categories and support coordination framework.
- Demonstrated experience working with participants with complex or changing support needs.
- Strong knowledge of participant choice and control, capacity building and person-centred practice.
- Experience communicating with the NDIA, Plan Managers, allied health professionals and disability service providers.
- Demonstrated experience in business development, relationship management, sales or referral generation.
- Ability to establish and maintain professional referral networks.
- Existing relationships within the NDIS, healthcare or community services sector will be highly regarded.
- Excellent written and verbal communication skills.
- Strong negotiation, networking and presentation skills.
- High level of organisation, time management and attention to detail.
- Ability to manage a participant caseload while also achieving business development objectives.
- Ability to work independently and as part of a collaborative team.
- Strong understanding of confidentiality, professional boundaries and conflict-of-interest requirements.
Highly Regarded
- Qualification in Community Services, Disability, Social Work, Psychology, Mental Health, Allied Health or a related discipline.
- Previous experience working as an NDIS Support Coordinator or Specialist Support Coordinator.
- Experience supporting participants with psychosocial disability, complex behaviours, high-intensity supports or SIL arrangements.
- Established professional network of Support Coordinators, Plan Managers, hospitals, allied health professionals and community organisations.
- Previous experience contributing to the growth of an NDIS or community services organisation.
Essential Requirements
- Current NDIS Worker Screening Check.
- Working with Children Check where applicable.
- Current National Police Check.
- Current Australian Driver's Licence and reliable vehicle.
- Full Australian working rights.
- Ability to travel for participant meetings, networking and stakeholder engagement when required.
