Our dedicated team of leaders and Disability Support Workers are committed to delivering high-quality care in a supportive and collaborative environment.

We are seeking an experienced, compassionate, and confident Home & Living House Lead Worker to join our team. This is a hands-on leadership role within our Supported Independent Living (SIL) services, supporting a participant with highly complex physical and psychosocial support needs, mental health conditions, and behaviours of concern.

This role is ideal for someone who enjoys leading by example, mentoring others, and remaining actively involved in direct participant support while ensuring the smooth day-to-day operation of the home.

About the Role

As a House Lead Worker, you will provide high-quality direct support while coordinating shifts, mentoring Disability Support Workers, and promoting best practice across the team. You will play a key role in ensuring participants receive consistent, safe, and person-centred support that aligns with their goals, NDIS plans, and behaviour support strategies.

Due to the complexity of the participants supported in this home, applicants must have demonstrated experience supporting individuals with complex mental health presentations, high-intensity support needs, and behaviours of concern. Experience implementing Positive Behaviour Support Plans and working within restrictive practice frameworks (where applicable) will be highly regarded.

Key Responsibilities

Deliver high-quality, person-centred support in accordance with NDIS plans, Behaviour Support Plans, and organisational policies.
Support participants with personal care, medication administration, daily living activities, community access, and skill development to maximise independence.
Provide support to participants with complex mental health conditions, psychosocial disabilities, high-intensity support needs, and behaviours of concern using trauma-informed and recovery-oriented practices.
Lead and coordinate day-to-day shift operations, ensuring continuity of care and effective communication between team members.
Act as the first point of escalation during shifts for operational matters, participant concerns, and emerging risks.
Coach, mentor, and support Disability Support Workers through positive role modelling and on-shift guidance.
Complete accurate shift notes, incident reports, medication records, and other documentation in line with NDIS Practice Standards.
Identify, manage, and escalate participant risks, behavioural incidents, and safeguarding concerns appropriately.
Foster a positive, collaborative team culture focused on participant safety, dignity, quality outcomes, and continuous improvement.

About You

To be successful in this role, you will have:

Demonstrated experience supporting participants with complex mental health conditions, psychosocial disabilities, and behaviours of concern.
Experience implementing Positive Behaviour Support Plans and applying proactive de-escalation strategies.
Strong understanding of person-centred, trauma-informed, and recovery-oriented practice.
Previous experience coordinating shifts or providing informal leadership within a disability or community services setting.
Excellent communication, problem-solving, and decision-making skills.
The ability to remain calm, resilient, and professional in complex or high-pressure situations.
A genuine passion for supporting people to achieve meaningful outcomes and live independently.
