About the role
An exciting opportunity exists for a Diabetes Clinical Nurse Consultant to gain experience in our HARP Diabetes Team. You will be providing diabetes self-management education to people living with diabetes who need extra support to manage their condition. A mixture of attending outpatients clinics, face to face consultations, phone consultations and home visits. If you like a challenge this is the opportunity for you. You will be primarlily located at the Footscray campus
Key responsibilities:

Support the diabetes clinic, see patients face to face, via telehealth and telephone as well as perform home visits (if required)

Support patients living with diabetes to achieve their best outcomes

Work collaboratively within the team
About you:

Tertiary qualification in Diabetes Management

Self-motivated and committed to lifelong learning
