About the role
As a CNHC Community Support Worker, will be engaged across multiple settings to provide and facilitate safe, responsive disability care and support for our clients living with disabilities in their homes and other community- based settings. This role will involve implementing care plans, on various levels of care support activities. The ideal candidate will excel in caring for individuals living with a disability with varying support needs to enhance physical, social, emotional, and intellectual development.
A Community Support Worker provides personalised assistance to individuals with diverse needs, helping them maintain independence and improve their quality of life. Responsibilities include personal care, mobility support, housekeeping, shopping, minor maintenance, and organising appointments. They also assist with medication monitoring, fitting medical aids, financial transactions, communication support, and community access. While tasks vary, the core focus remains on empowering individuals to lead fulfilling lives.
You will be employed on a full-time role and will have access to a company car if required.
Key Selection Criteria
Essential for the role:
- Relevant qualifications for Community services
- At least 2 years’ experience working in community support
- Experience in assisting in the implementation of support programs for clients in their home and community
- Experience with Complex Behaviours, Mental Health and Intellectual Disability
- Knowledge on restrictive practices and implementing behaviour support strategies
- Knowledge on Incident Reporting, Behaviour Charting and Reporting
- Has a Victorian driver licence and is confident in driving both an automatic and a manual modified vehicle- disability van
- Excellent communication and cultural sensitivity
- Working knowledge of digital rostering and CRM systems

- Current National Police Check
- First Aid & CPR certifications
- NDIS Worker Screening Clearance
