Care Coordinator (Care Partner)
Reporting to: National Program Manager
Location: Collins St. Melbourne VIC 3000
Key Responsibilities
- Conduct comprehensive client assessments to determine needs, goals, and risks.
- Develop, implement, and review individualised care or support plans.
- Coordinate and monitor the delivery of services and supports in line with client goals and funding arrangements.
- Work closely with clients, families, carers, and health professionals to ensure quality outcomes.
- Maintain accurate case notes, documentation, and compliance with regulatory and organisational requirements.
- Support clients in navigating My Aged Care, or other community support systems.
- Identify and respond to changes in client needs or risks promptly.
- Promote independence, dignity, and wellbeing through person-centred and strengths-based practice.
- Participate in team meetings, supervision, and ongoing professional development.
Skills and Qualifications
- Degree or Diploma in Nursing, Social Work, Community Services, Allied Health, or a related field.
- Previous experience in case management, care coordination, or community services.
- Sound knowledge of Aged Care, NDIS, or Support at Home programs.
- Excellent communication, problem-solving, and organisational skills.
- Ability to build trust and rapport with diverse clients and stakeholders.
- Current National Police Check, and NDIS Worker Screening Check (as required).
- Preferably with driver’s license and access to a reliable vehicle.
Pay: $33.00 – $37.00 per hour
