Right at Home Sydney Norwest is seeking an experienced, enthusiastic, highly motivated and reliable RN Care Coordinator to coordinate, manage and administer the delivery of care services to our clients. We provide services under a range of funding sources including private pay clients, My Aged Care and NDIS.

This will be a permanent full time position with Home Care Package and Budgeting experience Registered Nurse.

This role is responsible for:

Conducting client care assessments and care planning
Supervision and management of Care Staff
Intake processing, clinical care planning and reporting
Schedule appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements
Coordinate the provision of quality person centered care according to the client's needs, service/care plans and available funding in accordance with budget
Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, and diabetes
General administration with respect to tasks performed
On-call
Enthusiasm and building relationships to promote Right at Homes services

Job Types: Full-time, Permanent

Pay: $80,000.00 – $90,000.00 per year
