Hybrid permanent full-time role based at our Hervey Bay office

Access Support Coordination is looking for a dedicated and experienced Support Coordinator to join our growing team in Hervey Bay, QLD. In this full-time position role, you'll support participants in implementing their NDIS plans, ensuring they understand their options and make confident, informed decisions. You’ll play a key role in coordinating supports while promoting participant capacity, independence, and effective use of funding.

Key Responsibilities

Support participants to build meaningful and connected lives beyond funded supports
Assist participants in understanding and implementing their NDIS plans
Coordinate supports efficiently to maximise participant outcomes and funding utilisation
Help gather supporting evidence that reflects changing participant needs and future funding requirements
Identify opportunities in areas including employment, recreation, education, and housing
Advocate for participants and liaise with mainstream services to ensure responsibilities are met

About You

Proven experience in a Support Coordination role or similar within the Disability sector
Qualifications in Disability, Community Services, Social Work, or a related field
Strong knowledge of the NDIS and related processes
Ability to manage KPIs and billable hour targets effectively (highly regarded)
Excellent interpersonal and problem-solving skills with a participant-focused approach
Current driver licence, Working with Children Check, and NDIS Worker Screening Clearance required
