About Company

Care Fast Group is a registered NDIS provider supporting people with disability to live safely, meaningfully and as independently as possible in their homes and local communities. We are seeking a full-time Community Support Worker to join our team and provide practical, person-centred support to NDIS participants with varied support needs, including participants living in supported accommodation and community-based settings.

This role is suited to someone who can work beyond routine care tasks and actively contribute to each participant’s community participation, daily living goals, wellbeing, safety and connection with appropriate services.

About the role

As a Community Support Worker, you will work directly with participants, families, support coordinators, allied health professionals and internal Care Fast teams to understand participant needs and help implement practical support strategies. You will assist participants to access community, recreational, health, housing, employment, training and social opportunities in line with their NDIS goals and support plans.

You will also monitor participant progress, identify changes in support needs, maintain accurate records and escalate relevant matters to the appropriate team members so that services remain safe, responsive and person-centred.

Key responsibilities

The successful applicant will be responsible for:

Supporting participants to identify and work toward their personal, social, community access and independent living goals.
Assisting with the implementation of individual support plans, behaviour support strategies, risk management plans and daily support routines.
Supporting participants to access community services, recreational activities, appointments, training, employment pathways and other meaningful social opportunities.
Building respectful working relationships with participants, families, guardians, support coordinators, allied health professionals and other service providers.
Observing and reporting changes in participant presentation, wellbeing, risks, behaviours, support needs or service gaps.
Maintaining accurate progress notes, incident reports and other required documentation in line with organisational and NDIS requirements.
Assisting participants to develop daily living skills, community confidence, social connection and greater independence where appropriate.
Providing practical support in supported accommodation and community settings, including prompting, personal support, transport assistance and household routines where required.
Encouraging participant choice, dignity, rights, privacy and cultural safety in all aspects of service delivery.
Participating in team communication, handovers, reviews and service planning to ensure continuity of care.
Following Care Fast Group policies and procedures relating to safeguarding, restrictive practices, incident management, infection control, medication support, manual handling and workplace health and safety.

Skills and experience required

The successful applicant will ideally have:

A Diploma or higher qualification in Community Services, Disability, Mental Health, Welfare, Social Work, Case Management or a related field; or at least three years of relevant community services or disability sector experience.
Experience supporting people with disability, psychosocial disability, complex support needs, behavioural support needs or participants living in supported accommodation.
Understanding of the NDIS, person-centred practice, duty of care, dignity of risk and participant choice and control.
Ability to assess practical support needs, follow support plans and contribute to goal-based community participation.
Strong communication skills and the ability to work respectfully with participants, families, internal teams and external stakeholders.
Sound written documentation skills, including progress notes, incident reporting and handover communication.
Ability to work independently, use sound judgement and escalate risks or concerns appropriately.
Current NDIS Worker Screening Check or willingness to obtain one.
Current Working with Children Check or willingness to obtain one, where required.
Current First Aid and CPR certification or willingness to obtain.
Valid Australian driver licence and access to a reliable vehicle, where required for the role.

You will be a calm, reliable and values-driven professional who understands that community support is not just about completing tasks. You will be able to build trust, promote independence, support safe decision-making and help participants stay connected with the people, services and activities that matter to them.

You will be comfortable working with participants who may have complex communication, behavioural, psychosocial or personal care needs, while maintaining professional boundaries and following established support plans.
