Clinical Case Manager – Part-Time

Senior Helpers Whitehorse & Eastern Suburbs is looking for a caring, organised and clinically confident Clinical Case Manager to join our growing home care team.
This is a part-time role, starting at 2 days per week, 6 hours per day, with the opportunity to increase hours as the business continues to grow.
We are looking for someone who is passionate about supporting older Australians to remain safe, independent and well cared for in their own homes.
About the Role
The Clinical Case Manager will support the coordination and delivery of quality care for our clients, including clinical assessments, care planning, client reviews and ongoing monitoring of client needs.
You will work closely with clients, families, support workers, allied health professionals and external providers to ensure services are safe, person-centred and aligned with client goals.
Key Duties
Duties may include:
- Conducting client assessments and reviewing care needs
- Developing and updating care plans, support plans and risk assessments
- Completing clinical assessments and identifying changes in client health or wellbeing
- Monitoring client progress, incidents, falls risks and changing care needs
- Liaising with families, representatives, GPs, nurses, allied health providers and external services
- Supporting referrals for physiotherapy, podiatry, OT, nursing and other services
- Providing guidance to support workers regarding client care needs
- Assisting with onboarding of new clients
- Completing client reviews and documentation in line with quality and audit requirements
- Supporting compliance with aged care standards, policies and procedures
- Escalating clinical concerns appropriately and ensuring timely follow-up
- Helping improve systems, processes and service delivery as the business grows
About You

Essential Requirements
- Registered Nurse qualification
- Current AHPRA registration
- Experience in aged care, home care, community care, nursing or case management
- Confident completing clinical assessments, care planning and risk reviews
- Current driver’s licence and reliable vehicle
- Current police check or willingness to obtain
- Strong documentation and communication skills
- Ability to work independently and communicate well with clients, families and care staff
