Happy Days Support Service Pty Ltd is a growing disability support provider delivering high-quality, person-centred supports to children and adults living with intellectual, physical, neurological, psychosocial and developmental disabilities.

We are committed to empowering participants through education, skill development, community inclusion and individualised support that promotes independence and improves quality of life.

We are currently seeking an experienced Disability Services Officer to join our growing team to oversee program development and service coordination.

About the Role

The Disability Services Officer is responsible for assessing participant needs, developing and implementing individual support and capacity-building programs, coordinating services, monitoring participant outcomes and delivering person-centred disability supports across a variety of community and accommodation settings.

This position requires a professional who can work collaboratively with participants, families, allied health professionals and community organisations to ensure participants receive high-quality supports that promote independence, inclusion and achievement of NDIS goals. The role focuses on case management, program design, and service coordination rather than direct personal care.

Key Responsibilities

Assess participants’ individual needs, strengths and support requirements to design comprehensive service agreements and support plans.
Develop, implement and review individual education, life skills, training and support programs.
Interview participants and families to assess the nature and extent of support needs.
Monitor participant progress and maintain accurate case notes, reports and documentation.
Review participant outcomes and recommend modifications to support strategies where required.
Assist participants to access appropriate community, health, welfare, housing, employment and training services.
Liaise with families, support coordinators, allied health professionals, government agencies and community organisations to coordinate participant supports.
Promote participant independence, community inclusion and social participation through systematic program development.
Support participants in developing daily living, communication, social and life skills by training and guiding frontline support staff on individual support plans.
Coordinate and supervise the implementation of complex care plans, ensuring frontline staff adhere to established behavioural and lifestyle goals.
Provide direct disability support including personal care, community access, behavioural support and assistance with daily activities where required.
Implement and monitor Positive Behaviour Support strategies in accordance with behaviour support plans, providing guidance to operational staff.
Ensure services are delivered in accordance with NDIS Practice Standards, organisational policies and relevant legislation.
Contribute to participant reviews, goal planning and continuous improvement initiatives.

Essential Skills & Experience

The successful applicant will demonstrate experience in:

Assessing participant needs and identifying appropriate support strategies.
Planning, developing and implementing educational, life skills and support programs.
Monitoring participant progress and preparing professional reports.
Coordinating supports with families, allied health professionals and external agencies.
Supporting people living with intellectual, physical, psychosocial and developmental disabilities at a case-management or supervisory level.
Promoting participant choice, dignity, independence and community participation.
Building positive relationships with participants, families and stakeholders.
Working within a multidisciplinary and person-centred framework.
Maintaining accurate electronic documentation and records.
Applying excellent communication, problem-solving and organisational skills.

Essential Requirements

Advanced Diploma or Diploma in Disability, Community Services, Case Management, or a relevant higher education qualification.
Current NDIS Worker Screening Clearance.
Current Working with Children Check (Blue Card).
Current First Aid and CPR Certificate.
Current Australian Driver Licence.
Ability to work flexible hours including weekends.
