Case Manager – Adult & Older Adult Community Mental
Permanent – Full-Time
Based in Heidelberg
1x ADO per month
Open to Mental Health Professionals with following qualification: Registered Nurse (Div1), Clinical Psychologist, Social Work, Occupational Therapist
Member of the North East Area Mental Health Service (NEAMHS) Continuing Care Service (CCS)

About the division

The Mental Health Division is committed to promoting the highest levels of care for our patients. In realising this goal, the department will lead and excel in research, teaching and the use of person-centred evidence-based practice. The department provides services on all three campuses of Austin Health, from the Emergency Department through to bed-based services and ongoing ambulatory care.

About the role

CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. You will form solid working-relationships within the pre-existing community teams and foster strong therapeutic relationships with your clients & their families/carers.

You’ll join a friendly and supportive team who genuinely enjoy their work and the contribution they make.

A regular day will see you:
Assess and treat our clients, liaising as appropriate with family members and caregivers to promote patient care
Collaborate with multiple teams across the division and wider community
Support the care and recovery of peoples living with complex mental health diagnoses and presentations
Advocate for your clients right to access safe and timely care, at the right place at the right time

About you

You will be someone who has excellent communication skills and a strong commitment to excellence in your patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset

You’ll further have:
Excellent Mental State Examination and Risk Assessment skills
Minimum 3 years’ experience in a relevant mental health setting - including acute inpatient experience
Demonstrated knowledge of recovery and collaborative clinical practice.
A working knowledge of the Mental Health & Wellbeing Act (2022), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines
