Support Coordinator (Part-Time) – 10 to 15 Hours per Week

About the Role

We are seeking a motivated and organised Support Coordinator to join our team on a part-time basis. This role is ideal for someone who enjoys building relationships, problem-solving, and empowering participants to navigate and maximise their NDIS supports.

Key Responsibilities

Support participants to understand and implement their NDIS plans.
Connect participants with suitable supports and services, including Occupational Therapists (OTs), Physiotherapists (PTs), Allied Health Professionals, and community-based supports.
Conduct regular participant meetings and monthly participant visits.
Prepare professional monthly reports, progress updates, and case notes.
Build and maintain strong relationships with participants, families, service providers, and stakeholders.
Assist participants to build capacity, independence, and resilience.
Liaise with service providers to coordinate and maintain participant supports.
Develop community relationships and engage with industry stakeholders to promote services and support business growth.
Participate in networking opportunities, community events, and professional meetings.
Work collaboratively within a team environment while maintaining a high level of professionalism and accountability.
Maintain accurate records and ensure compliance with NDIS requirements and organisational policies.

Essential Requirements

Experience in Support Coordination, Community Services, Disability, Mental Health, or a related field.
Strong understanding of the NDIS and participant-centred practice.
Excellent communication and relationship-building skills.
A highly organised approach to work with strong attention to detail.
Professional attitude, presentation, and work ethic.
Ability to work independently while contributing positively to a team environment.
Current NDIS Worker Screening Check (or willingness to obtain).
Current National Police Clearance.
Current Driver's Licence and reliable vehicle.
NDIS Worker Orientation Module Certificate.

Desirable

Qualifications in Community Services, Disability, Social Work, Psychology, Counselling, or related fields.
Existing relationships within the NDIS sector.
Marketing, business development, or community engagement experience will be highly regarded.
