About the Role
As a Support Coordinator, you will work closely with NDIS participants, their families, and service providers to help participants understand and implement their NDIS plans while building their capacity to navigate the NDIS independently.
Your responsibilities will include:
- Coordinating and implementing participant supports.
- Connecting participants with suitable service providers and community services.
- Assisting participants to understand and utilise their NDIS funding effectively.
- Monitoring participant progress and reviewing support arrangements.
- Advocating for participants when required.
- Liaising with families, allied health professionals, plan managers, and other stakeholders.
- Maintaining accurate case notes and documentation.
- Ensuring services are delivered in accordance with NDIS Practice Standards and organisational policies.
Essential Requirements
- Previous experience as a Support Coordinator.
- Sound knowledge of the NDIS, funding categories, and relevant legislation.
- Excellent communication, organisational, and problem-solving skills.
- Ability to work independently and manage competing priorities.
- Current NDIS Worker Screening Check.
- Current National Police Clearance.
- Current Driver's Licence.
- Reliable, registered, and insured vehicle.
Desirable
- Relevant qualification in Community Services, Social Work, Disability, Human Services, or a related field.
- Experience working with participants with complex support needs.
- Experience using NDIS systems and participant management software.
