THE ORGANISATION:

Permalink Support Services is a registered National Disability Insurance Scheme (NDIS) Provider that offers Homecare & Community Care services, Coordination, Therapy, Housing, Supported Independent Living and Job Active Programs, to people with a disability.

Servicing all regions of Metropolitan Melbourne, Permalink is rapidly growing and expanding its care services with more than 500 Support Workers & Clients to date.

The Role:

Support Coordinator is one of the highest-ranking executives at Permalink Support Services, and their primary responsibilities includes client intake, management, business development, providing conflict resolution services, and acting as the main point of communication between the client, support coordinator/stakeholders, and our administration teams.

The role is being offered as an ongoing 38hrs/week, contract leading to full - time, Monday – Friday 9 am – 5 pm.

POSITION RESPONSIBILITIES:

As a Support Coordinator, you will be responsible for managing a portfolio of NDIS participants and ensuring they receive high-quality, person-centred supports that align with their goals and funding.

Your key responsibilities include:

Manage a caseload of NDIS participants and act as their primary point of contact.
Assist participants to understand and implement their NDIS plans using clear and accessible language.
Conduct participant onboarding, including completing service agreements and service bookings.
Develop strong relationships with participants, families, carers, support workers, and external stakeholders.
Coordinate and monitor supports to ensure services are delivered in line with participant goals and available funding.
Liaise with internal service delivery teams to ensure appropriate staff matching and effective rostering.
Monitor participant budgets and support utilisation to maximise outcomes.
Maintain accurate participant records, progress notes, reports, and documentation within organisational systems.
Manage incidents, complaints, and participant feedback in accordance with organisational policies.
Ensure compliance with NDIS Practice Standards, company policies, and relevant legislation.
Collaborate with internal teams to identify opportunities for service improvements and business growth.
Participate in mandatory training, team meetings, and continuous quality improvement initiatives.

SELECTION CRITERIA:

To be successful in this role, you will possess:

Essential:

Minimum 1 years' experience working within the disability, community services, health, or aged care sector.
Qualification in Disability, Community Services, Social Work, Health, Human Services, or a related field.
Demonstrated experience supporting participants within the NDIS or a similar person-centred environment.
Excellent communication and interpersonal skills with the ability to build strong relationships.
Strong organisational and time management skills with the ability to manage competing priorities.
High level of computer literacy and confidence using client management systems and Microsoft Office.
Excellent documentation, reporting, and administrative skills.
Strong problem-solving, negotiation, and decision-making abilities.
Ability to work independently while contributing positively within a team environment.
Current NDIS Worker Screening Check (or willingness to obtain).
Current Working with Children Check.
Current Australian Driver's Licence.
Reliable, roadworthy vehicle with appropriate insurance.
Full Australian working rights.

Desirable:

Previous experience in Support Coordination or Case Management.
Strong understanding of the NDIS, NDIS Practice Standards, and disability legislation.
Knowledge of participant budgeting, service agreements, and NDIS funding processes.
