With increasing client demand, workforce growth, and expanding community support programs, V & C Nursing Pty Ltd requires an experienced Community Worker to coordinate participant supports, facilitate community engagement initiatives, promote client independence, and ensure the continued delivery of high-quality, person-centred services that contribute positively to client wellbeing and community participation.

About the Role

The Community Worker is responsible for assessing client needs, developing and implementing individual support strategies, coordinating access to healthcare and community resources, monitoring client progress, and assisting individuals to maximise their independence, wellbeing, and participation within the community.

Working closely with clients, families, healthcare professionals, support coordinators, allied health providers, carers, and external agencies, the Community Worker plays an integral role in delivering holistic and person-centred support services that promote social inclusion, personal development, and positive client outcomes consistent with V & C Nursing Pty Ltd's service delivery model.

The position requires excellent communication, interpersonal, organisational, and problem-solving abilities together with a genuine commitment to improving the quality of life of individuals receiving healthcare, disability, and community support services.

Key Responsibilities

Conduct client interviews and comprehensive needs assessments to identify individual goals, strengths, risks, and support requirements.
Develop, implement, monitor, and regularly review individual support plans consistent with client needs and service objectives.
Support clients to develop independent living skills, social skills, confidence, and meaningful community participation.
Assist clients in accessing healthcare services, allied health supports, aged care services, housing assistance, education, employment programs, transport services, and community resources.
Facilitate client participation in community activities, recreational programs, life skills initiatives, and social inclusion programs.
Monitor client wellbeing, evaluate service outcomes, maintain accurate case notes, and prepare progress reports as required.
Provide advocacy, information, guidance, and practical assistance to clients and their families regarding available healthcare and community support services.
Liaise with healthcare professionals, support coordinators, allied health practitioners, carers, government agencies, and community organisations to coordinate integrated and client-focused service delivery.
Identify barriers affecting client independence and implement appropriate support strategies that promote positive outcomes and enhanced quality of life.
Maintain confidential client records, incident reports, service documentation, and case management records in accordance with organisational policies and legislative requirements.
Promote client rights, dignity, privacy, cultural inclusion, and person-centred practice at all times.
Participate in multidisciplinary meetings, quality improvement initiatives, and ongoing professional development activities.
Assist V & C Nursing Pty Ltd in maintaining compliance with workplace health and safety legislation, relevant healthcare standards, and organisational policies and procedures.
Support continuous improvement initiatives that enhance client satisfaction, service quality, and organisational performance.

Requirements

Australian citizens and permanent residents are encouraged to apply.
AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Disability, Human Services, Individual Support, Welfare, Social Work, or a closely related field, or at least three years of relevant experience may substitute for formal qualifications where applicable (ANZSCO Skill Level 2).
Demonstrated experience working within disability services, community services, aged care, healthcare support services, or related environments.
Strong understanding of person-centred practice, client rights, safeguarding principles, and community service delivery models.
Sound knowledge of disability support services, aged care services, community resources, and client-centred care practices.
Excellent communication, interpersonal, and relationship-building skills.
Strong organisational, case coordination, and problem-solving abilities.
Ability to maintain accurate client records and prepare professional reports.
Compassionate, empathetic, reliable, and client-focused approach to service delivery.
Ability to work independently and collaboratively within multidisciplinary teams.
Current National Police Check, Working with Children Check (where applicable), Current First Aid and CPR certification, or willingness to obtain.
Current Australian Driver Licence is desirable.

Work Conditions

Full-time, ongoing position.
