The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
We are seeking a skilled and compassionate Pregnancy Family Meeting Facilitator to lead a program supporting vulnerable pregnant women and families across the Nepean Blue Mountains Local Health District.
The Pregnancy Family Meetings (PFM) program brings together families and key agencies to support early engagement, collaborative planning, and positive outcomes for newborns who may be at risk of entering Out of Home Care at birth. Using a strengths-based and family-centred approach, the program empowers families to identify supports, build on their strengths, and participate in planning for the safety and wellbeing of their child.
As the primary Facilitator, you will coordinate and facilitate Pregnancy Family Meetings across NBMLHD, working closely with families, NSW Health services, and the Department of Communities and Justice. You will also contribute to the ongoing development of the program by supporting a network of skilled facilitators and fostering strong interagency partnerships that promote effective collaboration and service delivery.
This role is ideal for an experienced professional with strong facilitation, communication, and stakeholder engagement skills, alongside a passion for supporting families and improving outcomes for children.
