Join Western Sydney Local Health District's Community Health team, providing specialist social work services across Mount Druitt Community Health Centre and Blacktown Community Health Centre. In this role, you will:
- Work within an experienced and supportive Allied Health team
- Provide specialist care to clients with complex psychosocial needs
- Mentor and support social work staff and students to build workforce capability
- Contribute to service development, quality improvement and clinical excellence
- Deliver services across multiple community health locations within WSLHD
What you'll be doing
As a Senior Social Worker, you will provide advanced clinical social work services to clients, families and carers within a community health setting. Drawing on your highly developed clinical expertise and professional judgement, you will manage complex cases autonomously while providing leadership, consultation and support to colleagues across the service.
Key responsibilities include:
- Deliver expert social work assessment, intervention and care planning for clients with chronic, complex and palliative care needs
- Manage complex psychosocial issues including grief, trauma, loss, child protection, domestic violence, elder abuse and guardianship matters
- Provide clinical leadership, mentoring and professional support to social work staff and students
- Lead and contribute to quality improvement initiatives, service planning, education and evidence-based practice activities
- Collaborate with multidisciplinary teams, health professionals and external agencies to achieve optimal client outcomes

Essential Criteria:
- Qualification: Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work
Selection Criteria:
1. Degree in Social Work with eligibility for membership of the Australian Association of Social Work with demonstrated advanced clinical knowledge, skills and experiences in the provision of community Social Work, including palliative care.
2. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
3. Experience or capability in developing and implementing policies and procedures for service improvement, and in providing training and education with experience in staff and/or student supervision.
4. Well-developed time management skills with excellent written and verbal communication, including report writing skills.
5. Demonstrated experience in quality assurance and / or clinical practice improvement activities.
6. Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
7. Good computer skills, including experience using Cerner eMR or equivalent.
8. Drivers license valid within Australia and willingness to use for work purposes.
