Key Responsibilities include:

· evaluating client needs and designing, coordinating and delivering tailored support, education and training programs

· conducting interviews to determine the scope and seriousness of client challenges

· tracking client outcomes and preparing regular progress updates

· directing clients to suitable organisations for specialised assistance

· identifying community needs and reviewing the availability of services for health, housing, employment, education, and welfare

· collaborating with local groups, service providers, government departments and businesses to raise awareness and address community concerns

· supporting families and delivering care for children and people with disabilities in group homes, care facilities and service centres

· helping young individuals address social, emotional, and financial difficulties

· drafting funding proposals, resource applications and formal reports for government and other stakeholders

Requirements:

A relevant diploma or higher qualifications with specialisation in community services.
Experiences within a community/home care/residential care/disability care/youth work setting desired.
Outstanding interpersonal and communication skills.
Ability to commute to client homes and different residential facilities.
Valid driver's license and access to dependable transportation.
Police Check.
Up to date with Flu & COVID-19 Vaccinations
