Qualification / Study Requirement
As this position is intended to support the development of our Support Coordination team, the successful candidate must have a professional background in Social Work, or be currently undertaking a Bachelor's or Master's degree in Social Work in Australia.
Horizons is specifically seeking to develop a team member from the Social Work field who can progressively build practical experience in the NDIS and Support Coordination. This professional or academic background will also be an important consideration for potential progression from the Support Coordination Assistant role into a Support Coordinator position.
Key Responsibilities
Your responsibilities would include:
- Undertaking training and developing practical skills in Support Coordination under supervision.
- Assisting with follow-up and communication with NDIS participants, families, providers and other relevant stakeholders.
- Monitoring and following up NDIS-related documentation, including Service Agreements, Consent Forms and Support Plans for participants receiving Support Coordination and Support Work services.
- Researching suitable providers and assisting with referral processes according to participants' needs, preferences and goals.
- Supporting strategies to promote Horizons' NDIS services and strengthen relationships with other organisations and service providers. This may include organisational visits, emails, phone calls and other networking or engagement activities.
- Providing some Support Work hours when required, according to organisational and participant needs.
- Supporting general administrative and coordination activities associated with the delivery of NDIS services.
Conflict of Interest
Horizons is committed to maintaining appropriate professional boundaries and managing potential conflicts of interest. For this reason, you would not provide Support Work to any participant for whom you are directly involved in Support Coordination activities. Support Work would only be provided to other participants where there is no actual or perceived conflict of interest.

As Horizons is seeking to strengthen its Support Coordination capacity with professionals from the Social Work field, successful completion or progression of your Social Work studies, together with satisfactory performance, demonstrated competency in Support Coordination and organisational needs, may provide a pathway to progress from Support Coordination Assistant to Support Coordinator.
This progression may include taking greater responsibility for managing a caseload and, in particular, supporting English-speaking NDIS participants. Any progression would be assessed based on your qualifications, demonstrated skills and experience, performance, professional development and Horizons' operational requirements at the relevant time.
As part of our longer-term workforce planning, Horizons may also consider supporting an employer-sponsored Subclass 482 visa, provided that the future position, your qualifications, professional experience and individual circumstances meet the applicable migration, employment and sponsorship requirements at that time.
