Street to Home Program (Housing Support)

Based at The Beacon and undertaking outreach across the Perth Metropolitan areas

Every person who comes through the Streets to Home program is seeking safety, stability and a chance to move forward. As a Case Worker, you become a steady, compassionate presence during one of the most challenging periods in someone’s life. You offer practical support, advocacy and genuine human connection that helps people regain confidence, strengthen their wellbeing and take steps toward long-term housing.

Your work directly supports The Salvation Army’s mission to care for people, build healthy communities and work for justice. The impact you make is real, immediate and deeply meaningful.

About the role

As a Case Worker within the Street to Home program, you will support people experiencing homelessness through a mix of onsite work at The Beacon and outreach across Perth and surrounding areas. You will meet people where they are, providing person-centred, wrap-around support that helps them navigate the challenges contributing to homelessness.

A key part of this role is supporting people to acquire stable accommodation and sustain their tenancies, helping them build the skills, confidence and connections needed to maintain long-term housing and independence.

This is a permanent full-time position within the Homelessness Services WA team, reporting to the Team Leader Community Outreach. Flexible start and finish times are available between 8am–9am and 4pm–5pm. Pay is in accordance with a SCHADS Level 5.

How you will make an impact

Assessing service user eligibility from internal and external referrals

Completing program sign-ups and required documentation

Creating and maintaining client profiles, recording interactions and updating housing status

Ensuring service users are listed on priority public housing waitlists and in accordance with the Departments of Housing and Works’ National Partnership Agreement on Homelessness.

Developing collaborative case plans using evidence-based tools

Providing holistic case management including assessment, planning, facilitation, advocacy and review

Supporting service users to identify issues contributing to homelessness and linking them to appropriate services

Assisting individuals to build independent living skills to maintain future tenancies

Supporting people to acquire stable accommodation and sustain their tenancies through ongoing case work and practical guidance

Attending multi-agency case planning meetings where appropriate

Maintaining accurate records in TSA systems and meeting reporting requirements

Undertaking risk assessments to ensure worker and service user safety

Contributing to a positive, inclusive team environment and participating in meetings and reviews

Supporting implementation of the Homelessness Stream Model of Care

Complying with Work Health and Safety requirements and reporting incidents or hazards promptly

What you will bring

Minimum Diploma in Social Welfare, Community Services or related discipline

Relevant experience in community or welfare services

Knowledge of trauma impacts, mental health disorders and intervention models

Understanding of lifestyle pressures faced by disadvantaged community members

Ability to utilise assessment tools and undertake ongoing evaluation of individual needs

Knowledge of recovery-oriented case management

Ability to respond appropriately to challenging behaviours and high-risk situations

Strong communication skills and ability to build respectful, professional relationships

Initiative, discretion and sound judgement in daily duties

Commitment to diversity, inclusion and culturally safe practice

Resilience, adaptability and strong self-management skills

Understanding of homelessness causes, impacts and relevant legislation and standards

Valid WA employee Working with Children’s Check

First Aid Certificate (or willingness to obtain)

Current WA Drivers Licence
