Client Intake Coordinator – Enable Horizon Disability

Location: Armadale, WA

Enable Horizon Disability is seeking a compassionate and organised Client Intake Coordinator to join our growing team.

This role is ideal for someone with experience in the NDIS or disability sector who enjoys helping participants and building relationships with families, support coordinators and healthcare professionals.

Responsibilities

Manage new participant enquiries and referrals
Conduct intake interviews and onboarding
Liaise with Support Coordinators, hospitals and allied health professionals
Maintain accurate client records
Schedule services and support staff
Ensure compliance with NDIS requirements
Deliver outstanding customer service

Essential Requirements

Experience in the NDIS, disability, community services or healthcare sector
Excellent communication and organisational skills
Current National Police Check
NDIS Worker Screening Check (or willingness to obtain)
Working With Children Check (if required)
Current Driver's Licence
Full Australian working rights
