Care Partner (Support at Home)

Location: Melbourne, Victoria
Employment Type: Full-time / Part-time / Casual

About the Role

Great Care Pty Ltd is seeking a compassionate and organised Care Partner to support older Australians receiving services under the Australian Government's Support at Home program. As a Care Partner, you will work closely with participants, their families, and service providers to coordinate high-quality, person-centred care that promotes independence, wellbeing, and choice.

Key Responsibilities

Conduct participant assessments and develop person-centred care plans.
Coordinate and monitor the delivery of in-home support services.
Maintain regular contact with participants to review their goals and changing needs.
Liaise with families, health professionals, and community service providers.
Maintain accurate and timely documentation using electronic client management systems.
Ensure services are delivered in accordance with the Aged Care Act, Strengthened Aged Care Quality Standards, and organisational policies.

Essential Requirements

Qualification in Nursing, Social Work, Community Services, Aged Care, Allied Health, or a related field.
Experience in aged care, care coordination, case management, or community services.
Excellent communication, organisational, and problem-solving skills.
Current Australian Driver's Licence and access to a reliable vehicle.
Current National Police Check and NDIS Worker Screening Check (or willingness to obtain).
Strong computer skills and experience with electronic client management systems.
