Care Partner (Support at Home)

About the Role
Great Care Pty Ltd is seeking a compassionate and organised Care Partner to support older Australians receiving services under the Australian Government's Support at Home program. As a Care Partner, you will work closely with participants, their families, and service providers to coordinate high-quality, person-centred care that promotes independence, wellbeing, and choice.
Key Responsibilities
- Conduct participant assessments and develop person-centred care plans.
- Coordinate and monitor the delivery of in-home support services.
- Maintain regular contact with participants to review their goals and changing needs.
- Liaise with families, health professionals, and community service providers.
- Maintain accurate and timely documentation using electronic client management systems.
- Ensure services are delivered in accordance with the Aged Care Act, Strengthened Aged Care Quality Standards, and organisational policies.
Essential Requirements
- Qualification in Nursing, Social Work, Community Services, Aged Care, Allied Health, or a related field.
- Experience in aged care, care coordination, case management, or community services.
- Excellent communication, organisational, and problem-solving skills.
- Current Australian Driver's Licence and access to a reliable vehicle.
- Current National Police Check and NDIS Worker Screening Check (or willingness to obtain).
- Strong computer skills and experience with electronic client management systems.
