About Darling Daughters

Darling Daughters is a registered NDIS provider and aged care service provider committed to delivering high-quality, person-centred disability support services. We pride ourselves on providing safe, compassionate and evidence-based care that empowers participants to live independently while maintaining their health, wellbeing and dignity.

The Registered Nurse – Clinical Intake & Care Coordinator is responsible for providing clinical leadership and oversight to support the safe delivery of services across Darling Daughters.

This role leads participant clinical intake, nursing assessments, medication management, care planning, clinical reviews and ongoing coordination with healthcare professionals. The Registered Nurse will also provide clinical guidance to support workers, contribute to quality and compliance activities and ensure all clinical services meet NDIS Practice Standards and relevant legislation.

Key ResponsibilitiesClinical Intake

Conduct comprehensive clinical intake assessments for new participants.
Review referrals, NDIS plans, hospital discharge summaries and medical history.
Identify participant health needs, risks and support requirements.
Determine clinical suitability for service commencement.
Develop participant clinical profiles and recommend appropriate supports.

Nursing Assessments

Complete comprehensive nursing assessments.
Assess participant health status and monitor changes in condition.
Undertake assessments relating to wound care, continence, catheter care, diabetes, dysphagia, medication management and other complex health needs.
Recommend appropriate clinical interventions and supports.

Medication Management

Review medication charts and medication administration records.
Develop and review Medication Management Plans.
Liaise with General Practitioners and pharmacies regarding medication changes.
Monitor medication compliance and investigate medication-related incidents.
Provide medication guidance and education to support workers.

GP and Specialist Liaison

Coordinate participant care with General Practitioners, specialists, pharmacists, hospitals and allied health professionals.
Obtain clinical recommendations, treatment plans and medical documentation.
Ensure effective communication between healthcare providers and the organisation.

Care Planning and Reviews

Develop, implement and review:
Nursing Care Plans
Health Support Plans
Medication Management Plans
Clinical Risk Assessments
Emergency Management Plans
Review participant care plans regularly or when there is a change in health status.
Ensure all clinical documentation remains current and compliant.

Clinical Governance

Support the organisation's clinical governance framework.
Monitor compliance with NDIS Practice Standards and organisational policies.
Participate in clinical audits and quality improvement activities.
Develop and review clinical policies and procedures.
Promote evidence-based nursing practice.

Staff Education and Clinical Support

Provide clinical advice and support to support workers and management.
Deliver education on medication management, infection prevention and clinical best practice.
Conduct competency assessments where required.
Support continuous learning and professional development within the team.

Incident Management

Review clinical and medication-related incidents.
Investigate clinical risks and recommend corrective actions.
Participate in incident reviews and contribute to continuous quality improvement.
Assist with reportable incident investigations where required.

Documentation and Compliance

Maintain accurate, timely and confidential clinical records.
Complete nursing documentation in accordance with organisational policies and legislative requirements.
Ensure all documentation complies with NDIS Practice Standards, AHPRA requirements and relevant legislation.

General Responsibilities

The Registered Nurse will:

Deliver safe, person-centred and evidence-based nursing care.
Maintain current AHPRA registration and continuing professional development requirements.
Work collaboratively with participants, families, healthcare professionals and internal teams.
Promote participant choice, dignity, independence and rights.
Maintain confidentiality and professional standards at all times.
Attend meetings, supervision and mandatory training as required.

Additional Duties

As part of a growing organisation, the Registered Nurse may be required to undertake additional duties that are within the scope of their qualifications, skills and experience, including:

Assisting with participant onboarding and service commencement.
Supporting quality assurance and compliance activities.
Assisting with NDIS audits and accreditation processes.
Reviewing and updating organisational clinical documentation.
Developing clinical resources, templates and procedures.
Participating in organisational projects and continuous improvement initiatives.
Providing clinical advice to management regarding participant care and service delivery.
Undertaking any other reasonable duties as directed by the Director that are consistent with the responsibilities of the role.

Essential Criteria

Current registration as a Registered Nurse with AHPRA.
Current National Police Check.
NDIS Worker Screening Check.
Working with Children Check (where applicable).
Current First Aid and CPR Certificate.
Current Australian Driver Licence and reliable vehicle.
Strong clinical assessment and care planning skills.
Excellent communication and documentation skills.
Ability to work independently and collaboratively.
Experience in disability, community, aged care or primary healthcare nursing.

Desirable Criteria

Experience within the NDIS sector.
Experience in Supported Independent Living (SIL).
Medication management and clinical governance experience.
Knowledge of NDIS Practice Standards.
Experience working with participants with complex health needs.

Hours of Work

Permanent Part-Time.
Flexible working arrangements available.
Some travel across Melbourne may be required.
Occasional attendance at participant homes, meetings and clinical reviews.

Remuneration

Permanent Part-Time
$45.00 per hour + Superannuation
Annual leave, personal leave and other entitlements in accordance with the Fair Work Act and applicable Award.
