Due to continued growth, we are seeking an experienced and passionate Support Coordinator to join our team.

As a Support Coordinator, you will work closely with NDIS participants, their families, carers, guardians, support workers, therapists, and service providers to help participants understand and implement their NDIS plans, build capacity, achieve their goals, and connect with the supports they need.

Key Responsibilities:

Assist participants to understand and implement their NDIS plans.
Coordinate supports and services to help participants achieve their goals and maximise their NDIS funding.
Build and maintain positive relationships with participants, families, carers, guardians, support workers, allied health professionals, and service providers.
Conduct participant home visits and community-based meetings to better understand participant needs and circumstances.
Support participants to build capacity, increase independence, and make informed decisions about their supports.
Advocate for participants and assist in resolving barriers to service delivery.
Prepare participants for NDIS plan reviews and support meetings.
Maintain accurate case notes, reports, service agreements, and participant records.
Monitor participant progress and identify opportunities to improve outcomes.
Ensure all services are delivered in accordance with NDIS Practice Standards and Euroa Care policies and procedures.

You are proactive, solution-focused, and comfortable working independently while managing a diverse caseload. You are confident engaging with participants in their homes and communities and are committed to delivering high-quality support coordination services.

Skills, Experience & Attributes

Previous experience working within the NDIS sector.
Previous Support Coordination experience is highly regarded.
Strong understanding of the NDIS, participant plans, and available supports.
Excellent communication and interpersonal skills.
Ability to build rapport and maintain professional relationships with a range of stakeholders.
Strong organisational and time management skills.
Ability to work independently and manage competing priorities.
Experience preparing reports, case notes, and participant documentation.
Strong advocacy, problem-solving, and conflict resolution skills.
Ability and willingness to conduct participant home visits and community meetings.
Professional, compassionate, and participant-focused approach.
A genuine commitment to improving participant outcomes and quality of life.

Qualifications, Checks & Licences

Essential

Relevant university qualification in Social Work, Psychology, Human Services, Nursing, Law, Occupational Therapy, or a related field.
Current Australian Driver's Licence.
Reliable motor vehicle with current registration and comprehensive insurance.
NDIS Worker Screening Check.
National Police Check.
NDIS Worker Orientation Module Certificate.

Desirable

Working With Children Check.
Current First Aid and CPR Certificate.
Experience supporting participants with complex needs, mental health conditions, or psychosocial disabilities.
Knowledge of NDIS Quality and Safeguards requirements.
Current Australian Driver's Licence.
Reliable motor vehicle with current registration and comprehensive insurance.
NDIS Worker Screening Check.
National Police Check.
NDIS Worker Orientation Module Certificate.
