Description:
This Opportunity

We have an exciting opportunity for a passionate and experienced Case Worker to join the Release Support team, providing case work supports to people leaving incarceration. You will be joining a highly experienced and supportive team, led by the Team Leader Reintegration and Manager Reintegration and Rehabilitation.

You will have high-level case management skills, and a passion to work within a reintegration service that provides access to a wide range of supports and services that assist people in successfully reintegrating into the community from the justice system. The Case Worker role provides support across both pre-release and post-release clients, and co-facilitates an engaging life skills group program within Bunbury Regional Prison.

Key Role Responsibilities

Provide case management supports to individuals and families.
Develop collaborative relationships with other external services, government and community agencies and network with key local and specialist support services.
Actively contribute to a positive team culture with a solution focused, client centred approach.

Essential requirements

Experience working in reintegration, homelessness, AOD, FDV, and/or community services.
Demonstrated experience in case management practices and strategies to support vulnerable people.
Demonstrated experience in developing relationships and working with people with complex needs.
Prepared to work in multiple locations in the South West
