Description:
We have an exciting opportunity for a passionate and experienced Case Worker – Youth Services to join our Youth Support team, providing case work supports to young people aged 15 to 25. You will be joining a highly experienced and supportive team, led by the Team Leader Youth Services and Manager Community Services.
You will have high-level case management skills and a genuine commitment to supporting young people through a trauma‑informed, strengths-based, client-centred approach. The Case Worker role provides flexible supports to young people with complex needs, assists them to build independence and stability, and works collaboratively with a wide range of services to ensure holistic, wrap‑around support. You will also contribute to group-based life skills and wellbeing programs designed to build confidence, capability, and connection.

Key Responsibilities
- Provide case management supports to young people aged 15 to 25, addressing needs through a Trauma Informed strengths-based, client-centred approach.
- Develop and nurture relationships with external services, government agencies, and community organisations to create robust support networks. Actively engage with local and specialist services to enhance resources available for clients.
- Foster a solution-focused, client-centred team culture by collaborating with colleagues, sharing knowledge, and participating in regular team meetings and professional development opportunities. Support continuous quality improvement to uphold high standards in service delivery
Essential requirements
- Tertiary qualification in a human service focused discipline; substantial experience, skills and expertise in case management and high level case noting in a community services position
- Demonstrated knowledge of evidence-based case management strategies and principles with experience in delivering case management services to vulnerable people.
- Demonstrated ability to work directly with vulnerable clients who have complex needs and achieve positive and sustainable outcomes.
- Demonstrated high level understanding and communication skills when working with vulnerable clients.
