What you'll be doing
As a Registered Nurse within Cardiac Rehabilitation, you will provide comprehensive nursing care and rehabilitation support to patients referred to the program. You will play a key role in patient assessment, education, exercise prescription and ongoing care coordination to support recovery and self-management.
- Assess, manage and support patients throughout their rehabilitation journey
- Deliver education, counselling and self-management strategies to patients and families
- Conduct exercise assessments, testing and tailored exercise prescription
- Coordinate referrals and communicate with GPs, cardiologists and allied health providers
- Promote patient participation and reduce barriers to accessing rehabilitation services
