Full Job Description

Deliver life-changing outcomes supporting the older people of Western Sydney.
Collaborate with multidisciplinary experts across NSW health networks.
Position open to Registered Nurses and Allied Health Professionals.

Assess, support and empower older people by creating tailored aged care plans that maximise independence and connect them with the services they need.

Employment Type: Temporary Full Time, Temporary Part Time (up to 30th June 2029)
Position Classification: Registered Nurse, Allied Health Professionals Level 1/2
Remuneration: Dependant on Qualifications - $76,364 - $111,427 per annum + super
Hours Per Week: up to 38
Location: Westmead Hospital - Western Sydney Local Health District

The Seniors Assessment Service, Western Sydney Local Health District, is committed to helping older people access the right care at the right time. Based in Westmead, you'll join a highly skilled multidisciplinary team delivering comprehensive aged care assessments across hospital and community settings.

In this team you will be working collaboratively with patients, families, carers and healthcare providers, The Aged Care Assessor performs a vital role in supporting positive ageing and improving health outcomes.

What you'll be doing

As a Clinical Aged Care Assessor (ACAT), you'll conduct comprehensive assessments to determine eligibility for Commonwealth-funded aged care services while developing tailored support plans that meet the individual needs of older people and their carers.

You will be required to:

Complete holistic aged care assessments considering medical, functional, cognitive, psychosocial, cultural and environmental needs.
Develop evidence-based support plans and facilitate access to residential aged care and community services under the Aged Care Act.
Collaborate with multidisciplinary colleagues and external service providers to coordinate seamless, person-centred care.
Maintain accurate clinical documentation and manage assessment data in accordance with legislative and organisational requirements.
Respond to referrals efficiently while achieving service performance indicators and maintaining high-quality assessment standards.

Essential Criteria

Tertiary qualification & current registration with AHPRA in Nursing, Physiotherapy, Occupational Therapy or tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Work or Speech Pathology with eligibility for membership of Speech Pathology Australia.

Selection Criteria

Demonstrated post graduate experience working with older people in an acute or community setting.

Demonstrated ability to work independently & in an effective manner encompassing appropriate initiative, organisational, problem solving & time management skills.

Proven written & verbal communication skills including demonstrated computer literacy.

Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.

Understanding or experience in the health & functional support needs of older people & their carers in the community setting.

Current Drivers Licence (minimum class C) valid within Australia & demonstrated competent driving ability/ experience.
