The Complex Care Navigator - Allied Health (Level 4) is an exciting new position open to Dietitians, Occupational Therapists, Physiotherapists, Social Workers and Speech Pathologists.
This position is based in the Complex Medical Inpatient Unit in new, state of the art facilities at the Children's Hospital at Westmead and will work collaboratively with the multidisciplinary team to facilitate comprehensive and coordinated allied health service provision for children with complex medical conditions and/or disability.
The role includes coordination and leadership of inpatient allied health care as well as case management and discharge planning to support transition to local health services, community based therapy, NDIS and support services.

Undergraduate Degree in relevant Allied Health Discipline ie. Occupational Therapy, Physiotherapy, Speech Pathology, Social Work & Dietetics. Has registration or eligible membership for professional association.

Extensive experience and specialist knowledge with advanced clinical reasoning skills in working with children/young people with complex medical conditions and/or complex disability in the inpatient setting.

Experience and demonstrated ability to work collaboratively and effectively in a leadership role as part of a complex multidiscipline team. Advanced skills in team coordination, collaboration with different stakeholders and communication, both written and verbal.

Significant knowledge and experience coordinating care between families, health services, NDIS and other services to support children with complex medical conditions and/or complex disability and providing reports or documentation to support this.

Excellent time management and organisation skills with demonstrated ability to manage a complex workload and function within a complex health care setting.

Demonstrated leadership in planning, implementing, evaluating and reporting on services including development of clinical innovations and initiatives.

Demonstrated ability to provide leadership and management of team members.
