HenderCare is excited to welcome a Client Support Partner to our existing team in WA. This is a meaningful role where you’ll make a real difference working closely with both our NDIS and Support at Home clients.

About the Role

As the Client Support Partner, you will work with new and existing clients to develop and implement care plans, support goal setting, and deliver strategies that promote independence, wellbeing, and positive outcomes through person-centered care. You will work with a mixed caseload of NDIS and Support at Home participants.

Your responsibilities will also include:

Developing, reviewing, and maintaining individualised care plans that reflect participant goals, preferences, and funding eligibility.
Working closely with clients, families, and support networks .
Conducting quarterly budget reviews and ensuring services remain within allocated budgets.
Undertake regular reviews of service agreements.
Performing risk assessments relates to client’s health, home environment, and personal safety.
Assisting participants in understanding their approved services, funding classifications, and rights within the relevant framework.
Promote HenderCare services to potential clients and referral sources to increase service capacity.

Our Ideal Candidate

We are looking for engaging, energetic, and passionate people who show a strong commitment to the overall growth of the organisation. Attention to detail, and having the capacity to absorb and apply new information and processes will allow you to succeed in this role.

Ideally you will bring experience working in case management in disability, mental health, healthcare, social services, or community services. Knowledge of funding programs, ability to manage budgets and develop care plans will be highly regarded.

You will also bring:

Relevant experience Case Management
Knowledge of relevant funding models or desire to learn
Prior experience working in an office or corporate setting is desirable
A sound knowledge of government and non-government health services
