About Us
Nepean Blue Mountains Local Health District (NBMLHD) is an exceptional environment to advance your career while enhancing your skills and expertise. The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Join our dedicated Out of Home Care Health Pathway Program team and make a meaningful difference in the lives of vulnerable children and young people aged 0 - 18 years. As a Clinical Nurse Specialist Grade 2 (CNS2), you will provide specialist nursing care and clinical leadership, undertaking health assessments, developing individualised Health Management Plans and coordinating care for children and young people living in statutory Out-of-Home Care.

Working within a multidisciplinary team, you will collaborate with carers, families, clinicians and partner agencies to improve health literacy, reduce barriers to care and support positive health outcomes. This role requires advanced clinical skills, sound judgement, strong communication abilities and a commitment to delivering safe, compassionate and person-centred care.

What you will bring to the role

Current registration as a Registered Nurse/Midwife Division 1 with AHPRA with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate - e.g. Child & Family Health and/or Paediatrics and/or Adolescent Nursing or equivalent

Demonstrated experience working with children and young people within a multi-disciplinary team environment, with a focus on child development, assessment and the creation and implementation of care plans and knowledge and understanding of the impacts of trauma on child development, including short- and long-term impact on health outcomes, and demonstrated experience implementing current best practice within a therapeutic setting

Demonstrated high level communication, liaison, networking and negotiation skills. Strong co-ordination/organisational skills, with demonstrated experience developing and implementing administrative systems that ensure the effective use of resources and that can be used to monitor progress and performance measures

Demonstrated capacity to provide effective nursing care that is person centered, family inclusive and culturally sensitive to people accessing health services, including a high level of understanding of Aboriginal and Torres Strait Islander cultures and strong awareness of the historical and ongoing impacts of colonization for children and young people in Out-Of-Home Care

Demonstrated ability to perform in a clinical leadership role and use critical thinking as it relates to the delivery of advanced evidenced based nursing care and a demonstrated commitment to professional development.

Ability to work independently with minimal supervision maintaining involvement within the wider multidisciplinary team, whilst maintaining high level time management, problem solving, interpersonal and written and verbal communication skills. Including the ability to meet tight deadlines within defined resources and capacity to anticipate, respond and resolve complex critical issues

Knowledge of NSW Health networks and the demonstrated ability to utilize and leverage those networks to support the health needs of children and young people in OOHC

Current and valid Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.

Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
