Harmonious Community Care is seeking an experienced, compassionate and highly organised Team Leader – Wellbeing Worker to join our team in Darwin, Northern Territory.
This is a leadership position for an individual who is passionate about supporting people with disability and has the skills to lead Support Workers while ensuring high-quality, safe, respectful and person-centred supports are delivered in accordance with participant goals, support plans and relevant NDIS requirements.
The successful candidate will play an important role in coordinating day-to-day support delivery, mentoring and supervising support workers, maintaining service quality, and working collaboratively with participants, families, Support Coordinators and other professionals.
About the Role
The Team Leader – Wellbeing Worker will provide direct wellbeing and disability support where required while also providing day-to-day leadership and guidance to the support team.
You will be responsible for ensuring that supports are delivered consistently, safely and respectfully, with a strong focus on participant choice, dignity, independence, safety and quality of life.
Key ResponsibilitiesTeam Leadership & Supervision
- Lead, coordinate and support a team of Support Workers and Wellbeing Workers.
- Provide day-to-day guidance, mentoring, coaching and supervision to team members.
- Assist staff to understand and consistently implement individual participant support plans.
- Monitor staff performance, attendance, punctuality and adherence to organisational policies and procedures.
- Identify training, development and support needs within the team and escalate these appropriately.
- Assist with staff induction and onboarding.
- Promote a positive, respectful, inclusive and professional workplace culture.
- Support effective communication between workers, participants, families and management.
- Assist with resolving day-to-day operational issues and escalate significant matters to management.
Participant Support & Wellbeing
- Provide high-quality, person-centred support to participants in accordance with their individual needs, goals and preferences.
- Support participants to develop and maintain independence and daily living skills.
- Assist with community participation, social engagement and meaningful activities.
- Support participants to access appointments, activities and community services where required.
- Provide assistance with personal care and daily living activities where included within the participant's support arrangements and within the worker's training and competency.
- Support participants to maintain routines and achieve goals identified in their support plans.
- Promote participant choice, control, dignity, privacy, rights and independence.
- Recognise and appropriately respond to changes in a participant's wellbeing, behaviour or support needs.
- Maintain appropriate professional boundaries at all times.
Quality, Safety & Compliance
- Ensure support delivery is consistent with participant support plans, risk assessments and relevant organisational procedures.
- Promote a strong culture of participant safeguarding and zero tolerance for abuse, neglect, exploitation, violence and discrimination.
- Identify and report hazards, risks, incidents, injuries, safeguarding concerns and changes in participant circumstances.
- Ensure incidents are documented and escalated in accordance with organisational procedures and NDIS requirements.
- Maintain accurate, objective and timely progress notes, shift notes and other required documentation.
- Protect participant confidentiality and privacy and comply with applicable privacy requirements.
- Ensure records are maintained accurately and securely.
- Follow work health and safety requirements and report workplace hazards promptly.
- Support continuous improvement activities and contribute to quality assurance processes.
- Participate in audits, reviews, meetings and other quality and compliance activities as required.
- Ensure support workers operate within their scope of practice, qualifications, training and demonstrated competencies.
Communication & Coordination
- Maintain professional communication with participants, families, Support Coordinators, allied health professionals and other relevant stakeholders.
- Communicate participant-related concerns promptly to the appropriate manager or professional.
- Participate in participant reviews and team meetings where required.
- Assist in coordinating staff coverage and continuity of supports.
- Provide clear handovers and ensure relevant information is communicated appropriately between team members.
- Maintain professional and respectful relationships with all stakeholders.
Essential Requirements
To be considered for this position, applicants must have:
- Previous experience in disability, community services, aged care, mental health or a related human services environment.
- Demonstrated experience in a team leader, senior support worker, coordinator or supervisory role.
- Strong understanding of person-centred disability support.
- Demonstrated understanding of participant rights, choice and control.
- Strong written and verbal communication skills.
- Ability to maintain accurate and professional documentation.
- Ability to work independently and effectively lead a team.
- Ability to manage competing priorities and respond appropriately to changing participant and operational needs.
- Strong understanding of professional boundaries, confidentiality and safeguarding.
- Current NDIS Worker Screening Check, where required for the role.
- Current Working with Children Check, where the position involves working with children.
- Current First Aid Certificate.
- Current CPR Certificate.
- Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
- Valid Australian working rights.
- Current Australian driver's licence.
- Reliable and appropriately insured vehicle where participant transport is required.
A Certificate III/IV or Diploma in Individual Support, Disability, Community Services, Mental Health, Youth Work, Allied Health Assistance or another relevant qualification will be highly regarded.
Highly Desirable
The following experience or qualifications will be advantageous:
- Experience working with NDIS participants.
- Experience supporting people with autism, intellectual disability, psychosocial disability or complex support needs.
- Experience implementing participant support plans and behaviour support strategies.
- Experience supervising disability support workers.
- Knowledge of NDIS Practice Standards and participant safeguarding requirements.
- Experience with incident reporting, risk management and quality assurance.
- Experience coordinating staff rosters and participant supports.
- Experience working collaboratively with allied health professionals and Support Coordinators.
- Understanding of Positive Behaviour Support principles.
- Experience supporting participants with complex communication needs.
- Certificate IV or Diploma in a relevant field.
