The South Western Sydney Local Health District (SWSLHD) Youth Health Service is seeking an enthusiastic and experienced Social Worker to join our multidisciplinary team providing evidence-based, trauma-informed care to young people aged 12–24 years.
This is an exciting opportunity to work within a specialist youth health service supporting young people experiencing complex health and psychosocial issues, including mental health concerns, trauma, family conflict, homelessness, school disengagement, substance use, and other vulnerabilities.
As part of a highly skilled multidisciplinary team, you will work alongside Youth Health Nurses, Social Workers, Counsellors, Health Education Officers and Aboriginal Health Workers to provide holistic, integrated care across community, outreach and school-based settings.

About the role

The Social Worker will:

Provide comprehensive psychosocial assessment and evidence-based therapeutic interventions for young people aged 12–24 years.
Deliver trauma-informed, developmentally appropriate and culturally safe care.
Participate in multidisciplinary case planning and care coordination.
Undertake comprehensive risk assessments and safety planning.
Provide consultation and secondary support to Youth Health clinicians.
Develop collaborative relationships with schools, Child and Family services, Mental Health, NGOs and community partners.
Participate in clinical supervision, service development, quality improvement and evaluation activities.
Contribute to health promotion and early intervention initiatives across South Western Sydney

About you

We are looking for someone who:

Is passionate about working with vulnerable adolescents and young adults.
Demonstrates excellent assessment, formulation and therapeutic skills.
Has experience working with complex presentations including trauma, anxiety, depression, self-harm, family conflict and psychosocial adversity.
Enjoys working collaboratively within a multidisciplinary team.
Has highly developed communication, consultation and stakeholder engagement skills.
Can manage a diverse caseload while maintaining high clinical standards.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
Describe your experience providing psychosocial assessment and evidence-based therapeutic interventions to adolescents and/or young adults with complex mental health and psychosocial needs. Please include the therapeutic approaches you have used and the outcomes achieved.
This role works with young people who may have experienced trauma, family conflict, school disengagement, homelessness or other vulnerabilities. Describe how you apply trauma-informed, youth-centred and culturally safe principles in your clinical practice. Please provide a recent example.
Provide an example of a time you worked collaboratively within a multidisciplinary team and with external agencies (such as schools, health services or community organisations) to support a young person with complex needs. What was your role and what was the outcome?
This position requires excellent clinical judgement, risk assessment and case management skills. Describe a situation where you assessed and managed clinical risk for a young person. How did you prioritise care, communicate with stakeholders and ensure the young person's safety?
Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
