Service Coordinator – Job Description

Company: Enhance Hope Services Pty Ltd
Location: Perth, WA
Employment Type: Full-Time / Part-Time

Position Summary

The Service Coordinator is responsible for coordinating participant services and support workers to ensure safe, consistent and person-centred support in line with participant needs and NDIS requirements.

Key Responsibilities

Coordinate participant services, support workers and staff rosters.
Communicate with participants, families, guardians and other stakeholders.
Monitor service delivery, staff attendance and participant needs.
Maintain accurate participant records, agreements and service documentation.
Coordinate shift replacements and address staffing issues.
Follow up on incidents, complaints and participant concerns.
Ensure staff qualifications, screening and training requirements are maintained.
Liaise with Support Coordinators, Allied Health professionals and other service providers.
Support NDIS compliance, audits and continuous improvement.

Requirements

Experience in NDIS, disability, aged care or community services.
Strong communication, organisation and problem-solving skills.
Good understanding of person-centred support.
Good computer and administration skills.
NDIS Worker Screening Check and other relevant checks.
Certificate III/IV in Individual Support, Disability, Community Services or related field is desirable.

We are looking for someone who is reliable, compassionate, organised and committed to providing high-quality support to participants.

Pay: $35.00 – $45.00 per hour
