Role Summary

The Clinical Coordinator role works in close collaboration with the Facility Manager, Housing and Accommodation Coordinators, and the multidisciplinary team, it assumes clinical responsibilities across St Jude’s Disability Homes in both the Perth Metropolitan area and the Southwest Region. With a focus on enhancing the quality of life for clients with disabilities or psychosocial needs, the Clinical Coordinator role ensures the delivery of tailored support services, including high intensity supports, that meet the unique requirements of each individual residing in our accommodations throughout the metropolitan area.

Responsibilities and Duties

Ensures that the client assessments and evaluations are up to date, reviewed, reflected in their care plans and tailored to meet the clients care needs.
Ensures that our group homes meet compliance in all areas of clinical governance, including, but not limited to: Medication management, high intensity supports and compliance, Staff training as required, Clients have support plans in place and their care plans are completed, reviewed and up to date.
Advocates and exemplifies a person-centred approach to care delivery, rooted in comprehensive clinical assessments aimed at enhancing clients’ lifestyles and optimising their NDIS funded supports.
Provides supervision and mentorship to frontline and senior team members, ensuring the delivery of top-tier care that adheres to contemporary research findings and best practice standards.
Demonstrates positive role modelling and leadership in accordance with professional standards.
Efficiently coordinates all support services within your portfolio, while adeptly managing your team to ensure seamless operations.

Skills, Attributes and Experience

Essential requirements:

Registered Nurse with minimum 2 years’ experience.
Experience in Disability and Mental Health Sectors, preferably in residential and / or community setting.
Clinical expertise in the management of common medical conditions.
Understanding of quality & risk management systems and the Disability Standards Framework.
High level computer literacy in Microsoft Office and Excel.
Is organised and can effectively manage responsibilities and own priorities and assist others in this.
Excellent verbal and written communication skills.
Ability to develop staff, foster teamwork and effectively work as a team. member whilst providing leadership.

Desirable:

Understanding of a person-centred philosophy.
Ability to facilitate meetings and network with relevant stakeholders within the sector.
KPI driven and motivated.
Ability to identify, resolve and manage problems and resolve conflict situations effectively.
Ability to mentor new Team Leaders.
