About the role:
The role of the Gastroenterology Nurse Coordinator (GNC) is also at the forefront of the provision of hepatitis management for Western Health and has responsibility for the provision of high quality professional clinical care, needs assessment, health promotion, education and support for people living with hepatitis.
This will require the GNC to work collaboratively in a shared care model with the Hepatology Lead, treating specialists, multidisciplinary health teams, primary care partnerships and other key hospital and community-based services in order to provide care to both inpatients and outpatients.
Key responsibilities:

Demonstrate a sound knowledge of the clinical functions of Hepatology patient management and possess a commitment to remain informed of current innovations or changes in practice.

Provide education to patients, and their carers about hepatitis and the treatment programs.

Develop discharge plans that reflect the needs of patients and their significant others and demonstrate an understanding of the role of community providers.

Coordinate the shared care treatment model between specialist, primary care and community services and as the role develops provide an outreach service to facilitate the transition thorough the various levels of health care delivery.

Act as a clinical mentor to other staff by actively participating in sharing knowledge.
About you:

Current registration as a Registered Nurse with the relevant nursing regulatory body (e.g., AHPRA in Australia).

Experience in patient assessment, care planning, and coordination of multidisciplinary care.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal skills with patients, families, and healthcare professionals.

Ability to work independently and as part of a multidisciplinary team.

Proficiency in electronic medical records and clinical documentation systems.

Demonstrated experience in hepatology, gastroenterology, liver disease management, or a related specialty area (desirable).

Sound knowledge of chronic liver diseases, viral hepatitis, cirrhosis, liver transplantation, and associated treatments (desirable).

Experience in nurse-led clinics and care coordination programs (desirable).
