About the opportunity

To support clients to remain living in their own home through the provision of respite, personal care, domestic assistance and carer support in accordance with care plans and as directed by Care Advisors
To provide accurate and timely information about client needs/change in client condition to the Care Advisor.
Listen to clients and give them a voice, enabling them to choose for themselves.
Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers.
