Full Job Description

Ahanacare is looking for an experienced and exceptional Community Worker on a FULL-TIME basis to join our team at Morley, Western Australia, reporting to the Service Manager or Director.

About Ahanacare

We work directly with National Disability Insurance Scheme (NDIS) participants to help them live a more independent and fulfilling life. We are committed to providing our clients and their families with the support and services they require so that they can live with a sense of purpose, belonging and self-determination.

Ahanacare is a registered NDIS Provider to deliver the following supports:

· Development-Life Skills

· Daily Tasks/ Shared Living

· Assist-Life Stage – Transition

· Personal Activities

· Community Participation

· Support Coordination (including Specialist)

· NDIS Recovery Coach

Role and Responsibility

Interview clients to understand needs and develop community-oriented action plans; monitor and report on outcomes.
Undertake community needs assessments; identify gaps and priorities and refer clients to appropriate agencies for additional help if necessary.
Liaise with all members of the support/care team and provide coordinated care
Plan, deliver and evaluate group-based programs (life skills, participation, transitions)
Support and assist clients in life skills development, mental well-being and reaching individual goals in the community
Monitor and report on the client’s progress
Map services and maintain referral pathways; coordinate responses with community groups, welfare agencies, government bodies (including Service Australia and NDIS) and private providers.
Prepare funding submissions and required reports to stakeholders and government
Assist with the orientation of new staff and ongoing professional development of staff.
Maintain accurate records and data for quality and compliance.

Essential Skills and Qualifications

· Current First Aid certification

· Relevant advanced diploma or higher qualification in community service or community development

· Member of ACWA is desirable

· Demonstrated knowledge and skills in assessment, treatment and evaluation

· Demonstrated ability to work effectively in a multidisciplinary team setting as well as autonomously

· Ability to work with people from diverse backgrounds with compassion and empathy and without judgement for their lifestyle choices, personal preferences and values

· Demonstrated experience and knowledge of NDIS and working within a legislative framework

Mandatory Requirements:

Current NDIS Worker Screening Check.
Completion of NDIS Worker Orientation Module.
100-point Identity Check.

Desirable

· A can-do attitude, enthusiasm and great communication skills

· Strong, proven interpersonal skills and empathy will be found in the right candidate

· Excellent prioritisation and organisational skills

· Proficient in a 2nd language other than English
