Where you'll be working
The Transitional Aged Care Program (TACP) is a multi-disciplinary team supporting and providing aged care clients with the opportunity to improve their health and wellbeing after discharge from hospital. The service operates as a community-based service visiting patients within their own homes and comprises of Allied Health and Nursing professionals working closely with the clients and their families using an enablement focus, providing slow stream therapy aiming to improve the clients function, independence and safety. The team is part of Aged Care Services Lower Mid North Coast with Integrated Care – Hunter & LMNC in the town of Taree with beautiful beaches, countryside and boasts the option of seaside and rural living options.
You will be part of the broader Aged Care Services team under the umbrella of Integrated Care – Lower Mid North Coast that includes an Aged Care Assessment Services in both Clinical & Non-Clinical capacity, a Psychogeriatric/Dementia Support team, Aged Care Physiotherapist, Occupational Therapy and Podiatry community services.
The opportunity
Aged Care Services LMNC Transitional Aged Care Program (TACP) is looking for a Social Worker to provide a high-quality clinical service to clients/patients/consumers and their families/carers to ensure the provision of optimal outcomes for clients/patients/consumers of the service. The team has a TACP Coordinator and the team is managed by the NUM for TACP and Aged Care Assessment Services. You will be provided with onsite training to fulfil the role demands, all mandatory education is encouraged and supported within the team. The team is a close-knit group who conduct daily morning and afternoon huddles and a weekly case conference to keep abreast of all patient needs and updates.

The position will require the successful candidate to show skills such as self-motivation, autonomy, compassion, enthusiasm and good time management skills in order to complete tasks required each day. The services provided will be dependent on clients’ needs and will vary accordingly. Having some flexibility and the desire to work together as a team with great communication skills, both verbal and written, are required. Our ideal candidate will be committed to providing safe, compassionate person-centred care, aiming to assist the client to reach their functional goals.

- Demonstrated experience in working with complex and vulnerable patients, including Aboriginal and Torres Strait Islander clients
