Location: Blacktown & Mt Druitt Hospitals

Where you'll be working
Join a dedicated and supportive Renal Service delivering specialist social work across Blacktown and Mount Druitt Hospital services. Working alongside highly skilled multidisciplinary teams, you will play a pivotal role in supporting patients and families through every stage of kidney disease, from pre dialysis to transplantation. This is an opportunity to make a lasting impact while advancing your expertise within one of Western Sydney's largest and most diverse health districts.

What you'll be doing
As the Senior Renal Social Worker, you will provide advanced clinical social work services to patients living with chronic kidney disease and their families. You will use your specialist expertise to deliver complex psychosocial care, provide professional leadership and contribute to service improvement while supporting patients across the full continuum of renal care.

Deliver comprehensive psychosocial assessments, counselling and case management for renal patients and their families
Manage complex clinical presentations while advocating for patients and connecting them with health and community services
Provide specialist advice, mentoring and professional support to social work colleagues and multidisciplinary teams
Lead collaborative care planning, participate in case conferences and contribute to quality improvement initiatives
Promote culturally safe, person centred care while maintaining high standards of clinical documentation and professional practice

Essential Criteria

Eligibility for membership of the Australian Association of Social Workers (AASW)

Selection Criteria

Eligibility for membership of the Australian Association of Social Workers (AASW) with at least three years clinical experience.

Demonstrated high levels of clinical experience possessing extensive specialist knowledge in renal social work.

Experience working within a renal service supporting a wide range of renal patients and differing types of renal replacement therapies.

Experience in grief counselling and community supports for those impacted by chronic illness and increasing decline in health status.

A high level of interpersonal and communication skills including an ability to plan and co-ordinate client care independently as well as being able to operate effectively within a multidisciplinary team.

An understanding and knowledge of aboriginal social, cultural and economic issues; a capacity to flexibly develop local responses to community needs and cross-cultural communication skills.

Demonstrated effective verbal and written communication skills and familiarity with Microsoft Office suite.
