Full Job Description

North Coast Support Services is seeking experienced NDIS Support Coordinators to join our team as independent subcontractors.

This remote opportunity is suitable for self-motivated Support Coordinators who want flexibility, autonomy and the opportunity to build and manage their own participant caseload.

The role

The contractor will:

Help participants understand and implement their NDIS plans
Connect participants with appropriate funded, community and mainstream supports
Coordinate providers and multidisciplinary stakeholders
Monitor plan implementation and budget sustainability
Maintain accurate, compliant case notes and service records
Prepare progress reports and supporting documentation
Identify and respond appropriately to participant risks and safeguarding concerns
Build professional relationships and develop participant referrals
Manage their own availability and caseload

Caseload expectations

North Coast Support Services does not currently guarantee participant referrals, minimum hours or income.

Contractors are expected to develop and maintain their own caseload through professional networking and referral development. Once established, contractors should aim to deliver approximately 20 approved billable hours per week.

Any participant introduced to North Coast Support Services must freely choose the provider and enter into an appropriate service agreement.

Contractor remuneration

This is a genuine independent subcontractor arrangement.

The contractor receives 60% of revenue actually received by North Coast Support Services for their approved, compliant and billable services.
The current Level 2 Support Coordination rate provides approximately $60.00 to the contractor per approved billable hour.
Payment is made after North Coast Support Services receives payment for the relevant services.
No separate payment is provided for non-billable administration.
No minimum caseload, hours or income is guaranteed.
Initial 12-month subcontractor agreement with the opportunity for extension.

Essential requirements

Applicants must have:

Previous NDIS Support Coordination or relevant complex case-management experience
A sound understanding of NDIS plans, funding and service systems
Strong communication, coordination and professional report-writing skills
Capacity to work independently and maintain compliant records
A relevant qualification in social work, community services, disability, human services, nursing, allied health or a related field
Current NDIS Worker Screening Check
Current National Police Check
Working with Children Check where required
Australian Business Number
Appropriate professional indemnity and public liability insurance
Own computer, telephone and suitable private home-working environment

What North Coast Support Services provides

Flexible remote working

Participant referrals where available, although these are not guaranteed
Opportunity to develop a substantial independent caseload
