About the Role
As our Clinical Partnerships & Growth Manager, you will play a pivotal role in both supporting clients and driving business growth.
You will work closely with older Australians and their families to help them navigate the new Support at Home program, ensuring they receive services that enable them to live safely, independently, and with dignity in their own homes.
At the same time, you will be the face of our organisation within the community, developing and nurturing relationships with referral sources including:
- Hospitals and discharge planners
- General Practitioners and Practice Managers
- Allied Health Professionals
- Community organisations
- Retirement villages
- Aged Care providers
- Government and community agencies
This is a highly autonomous role where you will have the opportunity to shape the position, influence outcomes, and contribute directly to the growth and success of our organisation.
Key Responsibilities
- Conduct client assessments and reviews.
- Develop and monitor individualised care plans.
- Support clients to understand and maximise their Support at Home funding.
- Coordinate services that meet clients' goals and preferences.
- Build strong relationships with clients, families, and care teams.
- Ensure services meet quality and compliance requirements.
- Advocate for clients and support positive outcomes.
- Develop and implement local growth strategies.
- Build and maintain referral networks across the Orange region.
- Identify opportunities to increase awareness of our services.
- Attend networking events, expos, and community activities.
- Deliver presentations and education sessions to referral partners.
- Track referral activity and contribute to growth targets.
- Represent the organisation professionally within the community.
About You
You are a motivated Registered Nurse who enjoys both clinical engagement and relationship-building.
You will possess:
- Current AHPRA registration as a Registered Nurse.
- Experience in aged care, community care, home care, or care management.
- Strong understanding of aged care funding and services (or willingness to learn).
- Excellent communication and interpersonal skills.
- Confidence in networking and developing professional relationships.
- Strong organisational and time-management skills.
- Ability to work independently while contributing to a supportive team environment.
- Current driver's licence.
- Current First Aid & CPR
- National Police Check conducted within the previous 12 months

Pay: $100,000.00 – $110,000.00 per year
