Your Role
As a Care Coordinator, you will provide high-level care coordination and recovery-focused support to individuals experiencing complex health and substance use challenges. Working collaboratively with health services, community organisations and families, you will empower clients to access the right support at the right time.
Your Responsibility
Coordinate comprehensive, person-centred care plans for clients with complex support needs, including substance use concerns.
Driving a strong focus on developing culturally safe referral pathways for First Nations communities and strengthening connections with Aboriginal services and stakeholders.
Deliver strengths-based, recovery-oriented support aligned with Vinnies' AOD Model of Care.
Build strong partnerships with Local Health Districts, NGOs and community service providers to improve client outcomes.
Support clients to access services relating to housing, employment, education, social connection and community participation.
Conduct assessments, maintain accurate case documentation and participate in clinical reviews and quality improvement activities.
Manage referrals and intakes, working collaboratively with internal teams and external stakeholders to ensure timely access to support.
Represent CCCP at interagency meetings and support colleagues with complex case coordination across the service regions.
Deliver support through face-to-face appointments, telephone and MS Teams across region
You Will Need
Relevant tertiary qualifications or a VET sector qualification, together with significant experience in a related field.
Experience coordinating care for people with complex support needs, including Alcohol and Other Drug (AOD) concerns, co-occurring mental health conditions, homelessness, trauma, involvement with the criminal justice system, and family or cultural disconnection.
Experience undertaking comprehensive assessments and using standardised assessment tools to inform care planning and service delivery.
The ability to deliver therapeutic interventions and work effectively with clients to support positive recovery outcomes.
A professional, ethical and non-judgemental approach, with a commitment to person-centred and recovery-oriented practice.
A willingness to work within the Continuing Coordinated Care Program (CCCP), Vinnies' AOD Model of Care, and Client-centred Service Delivery Model.
Strong knowledge of community services and resources, with the ability to connect clients with appropriate supports.
The ability to work collaboratively within a multidisciplinary team, while also managing your workload independently.
Well-developed computer skills, including experience using client management systems and electronic data collection tools.
A current NSW Driver Licence and willingness to travel across Illawarra and Shoalhaven region
