About the role:
An exciting opportunity exists for a passionate and patient-centred Grade 2 Occupational Therapist to join the Western Health at Home team within the Rapid Allied Health (RAH) and Community-Based Transition Care Program (CB TCP) services.
The successful applicant will work collaboratively within a highly committed interdisciplinary team to deliver high-quality, efficient and innovative care to patients within their home environment. This role provides the opportunity to support patients across the continuum of care, helping to prevent avoidable hospital presentations, facilitate safe hospital discharge, and maximise independence for older people living in the community.
As a Grade 2 Occupational Therapist, you will contribute advanced clinical expertise, support service development initiatives, and provide clinical leadership and supervision to junior staff and Allied Health Assistants where appropriate.
Key responsibilities:
Provide Occupational Therapy Assessment and Intervention

Conduct comprehensive occupational therapy assessments and deliver evidence-based interventions for adults and older people receiving care in community and home-based settings.

Assess functional capacity, participation in activities of daily living, environmental barriers, falls risk, equipment needs and home safety requirements.
Promote Best Practice in Community and Aged Care Services

Apply advanced clinical reasoning and occupational therapy expertise to deliver person-centred care aligned with contemporary evidence-based practice.

Support safe discharge planning, hospital avoidance strategies and restorative care approaches that optimise independence and wellbeing.
Support Clinical Leadership and Staff Development

Provide supervision, mentoring and professional support to Grade 1 Occupational Therapists, Allied Health Assistants and students as required.

Contribute to education, quality improvement initiatives, service evaluation and continuous improvement activities.
Coordinate Care Within Multidisciplinary Teams

Collaborate with patients, carers, general practitioners, community providers and multidisciplinary healthcare professionals to develop, implement and review care plans.

Support patients to achieve meaningful goals while maximising safety, independence and quality of life within their home environment.
Deliver Services Across Western Health at Home Programs

Provide occupational therapy services across Western Health at Home programs, primarily within Rapid Allied Health (RAH) and Community-Based Transition Care Program (CB TCP), with flexibility to support other services across the continuum of care as required.

Contribute to innovative models of care that reduce avoidable hospital admissions and support sustainable discharge outcomes.
About you:
Occupational Therapy Qualifications and Registration
-
Appropriate recognised tertiary qualification in Occupational Therapy with minimum 2 years of clinical experience.

Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).

Eligibility for membership with Occupational Therapy Australia.
Community, Hospital and Aged Care Experience
-
Demonstrated experience in hospital, aged care and/or community-based occupational therapy settings.

Experience supporting older adults with complex health and functional needs is highly regarded.
Advanced Clinical Assessment and Intervention Skills
-
Demonstrated ability to independently complete comprehensive assessments, develop treatment plans and deliver evidence-based interventions.

Strong clinical reasoning, problem-solving and case management skills.
Leadership and Quality Improvement Capability
-
Demonstrated contribution to service development, quality improvement activities and implementation of best practice initiatives.

Experience providing supervision or support to junior staff and/or students is desirable.
Strong Communication, Organisation and Autonomous Practice
-
Highly developed interpersonal, written and electronic communication skills.

Excellent organisational and time management abilities with capacity to manage competing priorities.

Ability to work independently while collaborating effectively within an interdisciplinary team environment.
Additional essential requirements:

Commitment to ongoing professional development.

Current Victorian Driver's Licence.

Computer literacy and competency with electronic medical records and documentation systems.

Willingness to undergo a Police Check.
